Medical Home Family Stories

The concept of Medical Home applies to improving health care and health systems for all children, especially children with special health care needs. Combining the strengths of ethnographic approaches with new media, these prototype "family stories" aim to build capacity of physicians, families, and community agencies in partnering together to implement Medical Homes. Video family stories are designed for a variety of learning inter-professional contexts, including: learning on demand, mentor-based learning, and facilitated group discussions.
